Appointment Request/New Patients

We are currently NOT accepting new patients.  The American Medical Association (AMA) and all insurance carriers consider a patient “new” if they have not been seen by the practice in over three years.  With this in mind, please visit us regularly for annual physicals or  follow ups on chronic medical problems.
